Community Connectors

													

Community Connectors can assist people that are affected by many issues and concerns including loneliness, isolation and lack of confidence.

There can be many things that make people less confident about going out and getting involved in the community. Poor health, a change in circumstances or bereavement can affect our self-confidence and cause people to stop doing things they once enjoyed. Social Prescribing with Community Connectors can provide that conversation and encouragement that someone needs to get back on track and take part in activities, hobbies and interests.

Referrals can be made through any HSCP professional and can also be made online, by email, phone, post or in person. Community Connectors can visit people at home or a location that suits. We also accept self-referrals or referrals made by family etc.
